You only use .bat files for Total conversions. other than that you just place it in your custom stories folder and launch it through the game itself. For Total conversions make sure you put the .bat file in the root folder of the game.

To do it in the map editor use Block box's. They are invisible barriers. Entities ---> Technical ---> Block_box. Just use them to position him (Quite hard). You can use the model editor or 3ds max to do it too, but I have no experience in doing that.
